Diabetic Management Plan/Form
For the convenience of parents who have a diabetic child, a diabetic management plan and forms can be obtained from your health care provider at Children’s Hospital of the Kings Daughter and the Naval Medical Center Portsmouth diabetic centers. Should you have any questions about the diabetic management plan or form, please contact your school nurse.
